Requests that passengers when traveling with a service dog:

1. 1. Travel with the animal’s Veterinary Health Form or Immunization Record on your person
   2. While not required, they request passengers fill out the required Trained Service Animal PDF
   3. Upload it to My Trips through the Accessibility Service Request Form
